Introduction: The advancement of the COVID-19 epidemic in Brazil has threatened the functioning of the health system. Considering the limited health resources and the slow vaccination program, a risk assessment in hospital triage based on Brazilian data of patients of COVID-19 is primordial to improve the efficiency and guarantee the population access to the health system. The aim of this study was to evaluate the association between pre-screening clinical features (age, symptoms, and comorbidities) and utilization of health resources (use of ventilatory support or intensive care), and death related to COVID-19.
Methods: Patients from SIVEP-Gripe/MoH dataset cohort with positive laboratory or clinical COVID-19 diagnosis, aged 20-80, hospitalized, and dismissed in 2020 were included. We used logistic models with second order interaction between the effects of interest with logistic adjusted by confounders and p-values corrected by the Holm-Sidak method.
Results: Diabetes (27-65%), cardiovascular disease (24-38%), obesity (53-129%), and smoking (68-130%) increased the chance of respiratory support in almost all age strata and symptom groups, obesity (45-124%) was an important risk factor for ICU admission. Immunosuppression (70-235%), kidney disease (101-189%), and obesity (14-131%) increased the risk of COVID-19-related deaths. Patients presenting mild symptoms were more associated with the risk of negative clinical evolution.
Conclusion: The results revealed the complex distribution of risks among clinical features. The identification of high-risk patients during the hospital pre-screening would improve the efficiency of hospital medical resources, an indispensable condition considering middle-income countries such as Brazil and the unclear prospect of virus eradication.
