32709 - INEQUALITIES AND THE ROLE OF STERILIZATION IN MODERN CONTRACEPTION

Objective:
Our aim was to track the proportion of sterilization in modern contraceptive prevalence (CPMO) in low- and middle-income countries and its inequalities in terms of wealth.

Methods
Using data from national health surveys carried out since 2010 in 99 countries, we estimated CPMO, separating according to type of method (reversible or permanent). Where CPMO was at least 65%, we analysed wealth inequalities in the share of sterilization by wealth quintiles and Slope Index of Inequality (SII).

Results
We included 12 countries, among which the share of sterilization varied widely (from 1% in Zimbabwe to 60% in Dominican Republic). Sterilization was reported by more than half of women in Dominican Republic, El Salvador, Mexico, and Colombia. It accounted for less than 15% of CPMO only in Zimbabwe, Eswatini, and Paraguay. Most countries did not present important variations in terms of wealth. Higher levels of pro-rich inequality were found for Costa Rica and El Salvador (SII of 0.23 and 0.20, respectively). Sterilization was higher among the poorest only in Brazil (SII=-0.17).

Conclusions
Despite the wider range of methods available, several countries manage high levels of CPMO through sterilization. Sterilization might be recommended to some couples, however, its role on CPMO is excessively high in most countries analysed.
